Rigden Farm Enhanced Living/Supportive Services

Sometimes, all it takes to remain independent is a little help from day to day. Our Enhanced Living program provides personal assistance with reminders, errands, meals, household chores and care on an individual basis. These personal care services (listed below) are provided by a community-approved and licensed Home Health Agency, based on individual assessments by the Agency.

• Medicare certified home health care
• Medical assessment and care management
• Medical social work and home health aide services
• Personal care
• Medication assistance
• Complete on-site physical rehabilitation (Medicare-certified)
• Occupational and speech therapy (Medicare-certified)
• Monthly health screenings and blood pressure checks

Enhanced Living is designed so you use only the services you need and can change as you age in place.
